Case managers, nurse case managers and CMN medical directors work as a team to manage all aspects of a case from the initial referral until the patient returns home. We provide empathetic patient advocacy while monitoring costs.

This includes:

  • Selection of provider
  • Air Ambulance coordination
  • Coordination of admission
  • Pre-certifications
  • Monitoring the treatment plan to ensure patient receives appropriate care
  • Cost to date reporting
  • Discharge planning
CMN case managers not only monitor the care patients receive, but they also communicate clearly and regularly with the patient and family members. When necessary they provide logistical assistance for transportation and accommodation.

Patients and providers can call 24 hours a day, seven days a week for care management and coordination of referrals.
